I'm not much of a Hollywood fan, but my guess is that for some stunts you only get ONE "take".

Buster Keaton was an absolute madman. There's no way you could get away with stuff like that today.

You'd never see a Hollywood actor with hands that dirty either! Buster Keaton was a physical comedy genius... No choice until talkies came around!

In his later years, he went to get life insurance and had to have a full physical. The doctor asked when he had broken his neck. Keaton replied, "Never"
The doctor showed him the x-rays that indicated a broken neck at some point.
Keaton then mentioned that time he jumped off a bridge onto the last car of a moving train for stunt . . . but missed. He said his neck did hurt for a month or two after.
Ah! The 'good old days!'
