I used to work with a guy that was on a small crew running a logging machine. Not sure of the specifics. The machine caught fire and the fuel tank popped. A couple guys were standing near it and got hit by the fireball. One was badly injured immediately, but they were both singed black with soot. The one guy who wasn't injured was standing off to the side smoking cigarettes while the rescue crew arrived. Turns out they both ended up dying in hospital because their lungs filled with fluid.

Maybe there is no cause and effect here, but...
Spray it with water and it explodes!
Let's try spraying it with water again, What could go wrong?
