I don't see a button hole anywhere...
After the outline is sewn and the work removed from the machine it is then cut open with a knife or scissors
The newer digital sewing machines let you set various parameters for the button hole, like stitch density and width. If you can run a milling machine or a lathe for sure you can do buttonholes. Now on the other hand if you want to hand sew perfect buttonholes, well, this guy Maurice Sedwell is a good teacher:
