Have you had any problems with the cam carrier being too loose to allow a good bite with the cam?
If so, you might consider turning the cam carrier shaft, between the boss and thread, to 6 or 7 mm then cutting flats to fit the 4 mm slot.
At 13:28 when you made the mark for the lever hole, I thought you were choosing a spot to the lever would end up at a certain angle, but then I realized the whole carrier mechanism could rotate all the way around.
