    What's to keep it from going sideways and folding up while you are on it? Looks like an accident waiting to happen.

    Your weight on the rungs keeps it open and from folding up, because that notch on the top is a stop. Regardless of your design if your ladder goes sideways (unless it's a library one) you're in trouble :-)

    Nearly every fire truck in the country has something similar to this on it, only aluminum. They are called attic ladders. There is a folding latch on the bottom that keeps them from folding up while climbing them.
