    I think I saw a similar hydraulic cylinder being fixed by the low bid company in India, but they used a gantry crane to do all the work, and they had sandals on.
    Are those forks heat treated to be hard enough to snap off? You don't want them to bend, so they must be.

    A little Common Sense was needed hear to avoid this. As simple as using two slings, one on each fork and the load back as far as can be on the forks.

    It looks like they had fork extenders on them which slid off.
    Stupid is forever, ignorance can be fixed.

    Worked in the elevator industry for 35 years. When I did a jack change they were always delivered in pieces and we always used a Gantry System.

    During my 5 minute "certification" course on the forklift, they told me to always keep the load as close the the ground as possible.
    Seems that would have helped this guy out.

    Load too heavy for the capacity of the forklift, should have gotten a bigger forklift. Sling should have been as close to the truck as possible to reduce the affect of the weight of the load. Fork extensions looked like they just slid off, no fastener / stirrup - that's what flew up in the air. Not enough room to safely unload. Should have come just off the edge of the flatbed then straight to the ground and moved using different method. Driver was inexperienced - lowering the load as quickly as possible to the ground would likely have eliminated the upending. I bet the driver's back still hurts!
