I have used the ratchet part of a chest brace (that drill that carpenters used, don't know what the American name is) for various applications that need a drive in one direction and is needs a bit of force. It can be locked as well.

In the USA it's known simply as a "brace". With a screwdriver bit in it, it's still my preferred way to put a lot of torque on a stubborn screw while still having precise control.
