I suspect that inside is a helix that attaches to the socket. As the user turns the crank, a nut moves towards the socket end of the toop and rides down the helix. The pitch of this helix must be large in order to provide the needed mechanical advantage. With lots of grease on the surfaces, it should all work smoothly. Brilliant design!
