Not a malfunction. Main rotor snagged a steel cable attached to steel tower.

He was definitely wearing a seat belt. The extreme forces required to rip the gearbox off the airframe mounts would hurl the pilot and passenger clean out the machine if they had not been strapped in.

He gets thrown out at about the 8 second mark. You then see him getting up after the aircraft has come to rest. Amazingly he did not appear very injured. Unless that was part of the ground crew.

The chap getting up was the ground crewman just below the helicopter. The harness does indeed pull the pilot back into the helicopter. The pilot is concussed shown by only little movement from left seat Pax, compared to ground crew already standing up. There was only the pilot on board.
