I recall an early version, 70's-ish semingly in all the aircraft subcontractor shops. Logically, the artist contrived this illustrating frequent disconnects from engineering, the production shop, and assembly lines.
Thankfully, the chart progressed with new fixes. I'll refrain posting judgments on how real it now appears...
My favorites; the substantial sized fasteners hiding insufficient connections, and hex heads to twiddle while looking busy.
