Ladder not nearly vertical enough for that application, should be near vertical with the strap around the concrete base. A rope on the top rung hanging down the back side of the ladder as it is placed on the pole. Then the rope taken around the pole, and FLIPPED over the top rung and tied off at the bottom to secure the ladder at the top. This puts very little stress on the pole.

That ratchet strap from the ladder to the pole along with the low angle of the ladder caused that failure. It is such simple physics.
