the back piece is like the sections of a scoop stretcher. The piece supporting the patient's back is actually a metal piece which is attached to the side rail, not a continuious piece which spans from one side rail to the other. (think cantilevered)

the scoop stretcher I'm used to is made of aluminium and has connectable joints at the head and feet. the patient is rolled slightly to one side, and the left side placed in with the metal flags under the paitent, then the patient rolled to the right and the right side piece inserted.
once both pieces in place, the connectors at the head and feet are clicked together and the entire stretcher is rigid and secure.
