I blaim the driver.
One should check your vehicle and load before leaving the yard.
Report any death traps to your supervisor.
Ha Ha Ha, obviously doesn'y apply here though.

That "report to your supervisor" does not work in most places. You can bet the driver did not own that truck. He was trying to make money to feed his family. He was told to drive the truck. If he did not drive it, the next guy in line would take the job, and the first guy would not be asked back.
