Our second floor had precast slabs put down (7 meters wide) and forms just around the stairs. Rebar all around, of course. Then they poured the whole thing...and it stayed!
If I remember right, it's 20cm thick.

That looks like my concrete skills. Mine has never collapsed though.

Looks like the guy running the vibrator caused the column form to fail, which led to this domino failure of all the support forms.
