    Here's some information on a different but similar accident from 2006, from the California Fatality Assessment and Control Evaluation (CA/FACE) Program. This one also involved a worker being pulled into a warping machine in the Los Angeles area: California FACE Report #06CA006.

    Judging from the cleanliness of the floor Obviously safety is not the number one concern of this factory.

    That is my thought too. He's fell asleep.

    Yep gotta be on the ball around machines.

    Place where my father & I worked at have rolling mills with 2 large contra-rotating rollers. Of course if you get too close it will feed you in, then spit you out the other side as a 2 inch slab!!!

    So a chest height bar was installed. Trip it with your body and machine stops. Been a while, but think from memory it then reversed to push you back out. But anyhoo you get the general jist.

    Group of Indians (subcontinent) came round on a factory tour. The inevitable question about the bar came out. Their repsonse was, Oh we don't have that kind of thing over there!. Theres 50 blokes outside looking for a job...." Don't really need to spell the rest of that conversation out to the masses.

    yup falling asleep at the wheel is a real killer. safty means nothing in many countrys...less than my spellen.
