    The most common causes are - magazine spring not strong enough to push the next round up in time for the slide to pick it up and to help eject the fired case, the extractor not holding on to the fired case long enough, and the impulse of the fired ammo not pushing the slide back far enough due to a recoil spring too strong for that load. Semi autos have a materials handling problem that revolvers don't have.

    The semi (or full) auto has to get the round out of the magazine, into the chamber, and the fired case out of the chamber every shot, where the revolver leaves them in place the whole time, and just rotates them past the firing pin.
