What could a spotter do with a free weight lift? Get both of them injured.

reckon the end of the bar probably clipped his hair, if not his scalp, they way his hand went up so fast after the fall.

I see a need for a couple of ropes, or cables, running through a braking apparatus, which can be deployed, instantly.

    Not a bad idea.
When I was in high school the coach told everyone never to lift more weight than you can lower without dropping. If competitions required lowering the weights as well as lifting them to complete a full repetition in order to win the competition. They would be a lot more interesting. Probably 100 to 200 lbs lower totals but more interesting none the less.

The end of the bar definitely hit his head and bounced it off the floor. That had to hurt a lot!

I noticed it hit the gym floor instead of the plywood cushion mat. That probably didn't make the school board very happy having to have the floor repaired
