    Ok 2 questions. First why would an operator travel at speed with the forks high up in the air in the first place? And my second question is if there is a known discrepancy in a busy or even in a not so busy warehouse where this type of high reach minimal footprint forklifts are used, why are no safety hazard warning stripes painted on the floor to warn operators of the hazard.

    Much like most of these incidents, the root cause is nobody in the whole company has two IQ points to rub together.

    You are exactly correct, Companies are mostly run by the CFO. Never spend a dime on having any extra inventory on hand since that would mean $$$$ sitting on the shelves. Run a strict inflow to outflow based purely on the just in time delivery policy. This means employees are required to preform at a +100% capacity at all times to save on labor costs. The second an order is received it should have already been shipped. If a few safety precautions happen to be overlooked as long as the product is out the door before the ink is dry the company receives a higher profit margin, parish the thought of upper management not receiving a higher bonus than their previous bonus, after all they can't be expected to maintain their lifestyle on a mere salary which is only 2o times more than the mid level management receives can they?

    The CFO would probably say... "... just buy a new forklift, take it out of his paycheck. He said he knew how to operate those..."

    There it is again; Safety Third, exactly as Mike Rowe describes. I'd take it from his check, repairs or replacement because there is 100% evidence.
    But the "Said he knew..." stuff ended decades ago. Have to qualify on everything above electric pencil sharpener.
