    Something caused the cyclic stick to be pushed way forwards causing it to tip like that. Expensive landing!

    Pilot error, I'd bet. The approach was wonky, with the mast swinging like a weed in the breeze, and it looks like it got into a ground resonance problem. The mast tilts back right after the skids touch, then it goes fully forward and stays there.
