After negotiating the turn the axle set was not tracking true to the truck. the rear escort should have informed the driver of this that is one of the reasons they are there for not to just drive along with their lights flashing while talking to their girl friend boy friend wife or husband. You are there to help protect the load and warn the public.
Another thing I noted was the only securement for the beam was over the top nothing securing the bottom to the turn table of the steerable axle set except for the weight of the beam on the block of dunnage there should have been 4 chains attached to the bottom of the beam to secure the turntable to the flange in addition to the chains to the top flange those are only to keep the load secured vertical.
Just because a company or driver owns specialized equipment does not automatically make them over dimensional haulers

Don't know what the dolly ran over, but driver saw it; brake lights on that very instant.

Looks like the tail section, (not tracking as stated before), hit the concrete barrier on the right side. Driver saw it in passenger mirror but could not stop in time.

There are concrete barriers on the right side of the road. You can see them at the 0:32 mark between the beam and the dolly.

This is what happens when someone who does not know what he is doing, does it anyway.
It appears that the rear dolly rotated under the beam during the turn. It appears to be properly aligned before the turn. But a three axle dolly will have a strong tendency to run STRAIGHT: some of it's tires MUST skid sideways in a turn. The load was turning - the rear dolly wanted to go straight - there was no way near enough rigging to resist this - so the load rotated on top of that rear dolly. And coming out of the turn it did not track properly. The driver should have noticed and stopped. The rear escort also should have seen this and been on the radio screaming "STOP!"
This would make a great training film for the riggers, the drivers, the escort drivers, and the MANAGERS who hire low skilled people to do these things. There is plenty of blame to go around. Oh, and throw in the bean counter who wanted it delivered FAST.
