    There is only 2 ways I can think of that happened. which is #1 if he left the protective dust cover in place #2 he had allowed the muzzle to fill with dirt or debris fouling the rocket in the tube. there may be a 3rd way if there is a safety lock to keep the rocket positioned so the firing Triger can activate it which is supposed to break away or release once the trigger is pulled this could also have failed due to dirt.

    Similar to the 1/4" ball bearings inside random 5.56 rounds?

    Not every one just random casings formed after the powder and bearing installed.

    Can't trust any of the munitions left behind. They just might blow you up, instead of the enemy.
