    Brake mounting bolts are usually much smaller than axle bolts.
    If the axle is retained to the suspension with caps then I would suspect that the cap bolts either broke due to misuse or fell out do to insufficient installation.
    Hitting the brakes just stopped the wheel but the bike (no longer connected to the wheel) just kept going.
    The big question is: Did anyone learn anything or just pass the blame on to someone else?
    Skinned knees and gravel chin can be a great teacher, or be ignored. That is all on the student...
    I froze the video at 21 seconds, and you can see that there is more on the tire than just the hub. Looks like part of the front suspension is still there. Maybe a front-fork failure.
