If I had to guess, its a shaft element to permit attachment of a flexible shaft (with a knob at the far end) to control movement of a telescope tube (or some kind of alti-azimuth or rotary mounting system). I don't recall it has a specific name but the protruding part is simply half-round shaft section to allow for a really solid connection to a flexible shaft that is secured by a thumb screw.
