    Why trust a hook at all. It should have been eye shackles.

    They only had the hook over the edge of the flange. Very bad, I'm surprised it didn't fail sooner.

    The crane operator should have know better. Poor training all around.

    Any idea where this happened? Perhaps what make of truck?
