Why I have never thought about this, I can't say. What are the restrictions on cupholders or drinking a cup of coffee or whatever if you're driving professionally or militarily? I guess I've never seen a cupholder for a pilot in an airplane; what about military vehicles? Or commercial vehicles. Heavy equipment. If those vehicles are intentionally designed without cupholders, or if it's common in military situations to forbid drinking a beverage while driving (is it?) maybe that can tell us something.
I agree there is that weird moment when you're trying to balance the cup up to your mouth while also driving the car, and it is indeed distracting.
mklotz's joke (?) about the tube now doesn't seem so absurd, although of course the setup has to be correct for hot liquids.
What do racers do? Don't some of them use the straw method, with a steering wheel button? AFAIK, they dispose of the liquid from their body using such a system.
Now, if we want to go in the other direction, and do something more dangerous, here's an option:
