looks like the connecting bolts between the lower and the upper section had rusted away or someone had removed them. the single stand off was not designed to support vertical loads failing under the weight of 2 men. the ladder appears to have been rusted way beyond being serviceable anyway since many of the rungs broke away once it hit the ground.
I'm betting at least 1 possibly 2 or 3 broken legs a couple broken arms and more than a few cracked ribs.
I would be very surprised to hear there were no broken bones or more serious injuries after a fall from that height with the scrap metal landing on top
