I suspect that one of the cylinders was filled way beyond the 80% max, and in the heat of the day started outgassing on the pressure relief valve, then some dummy with a cigarette set the gas on fire.

I was on a job site where somebody brought the boom of a manlift down on the top of an acetylene cylinder. It took a few hours for that to completely gas out. Site evacuation. That was an interesting day.
