We had a guy cut a beam with a torch, then start beating on it with a sledgehammer to break the slag WITH HIS FEET UNDER IT. So when it let go it landed directly on his foot, resulting in a broke foot and a worker's compensation claim. We had a guy get dust in his eye, go to first aid, then while looking up complaining about the dust in his eye he took off his hard hat AND POURED MORE DUST INTO HIS EYE. We had a guy put up a 2x4, turn around, then immediately walked into the board he just nailed up, breaking his tooth. We had an open hole, so we had it taped off and a guy stationed there as hole watch. After a few hours guess who fell into the hole he was supposed to be watching. Many other examples. But the moral of the story is tools like that can't exist today. I'm not sure if people are dumber, but management now has to own their dumb.
