The only problem I had with making them is milling off the wrong side on the second cut. I had to go to the hardware store twice more than should have been required.
Some quality time with needle files is also required to get them operating smoothly.

You could have used one of these, they've been around for years: https://www.mcmaster.com/slip-on-nuts

I'm guessing you cross drilled a clearance hole through the coupler first. Then milled the appropriate portions of the sides away. I like the idea of a washer with a recess, too. Looks like a very handy setup.
